RESUMO

Accuracy of aortic regurgitation (AR) quantification by magnetic resonance (MR) imaging in the presence of a transcatheter heart valve (THV) remains to be established. We evaluated the accuracy of cardiac MR velocity mapping for quantification of antegrade flow (AF) and retrograde flow (RF) across a THV and the optimal slice position to use in cardiac MR imaging. In a systematic and fully controlled laboratory ex vivo setting, two THVs (Edwards SAPIEN XT, Medtronic CoreValve) were tested in a porcine model (n = 1) under steady flow conditions. Results showed a high level of accuracy and precision. For both THVs, AF was best measured at left ventricular outflow tract level, and RF at ascending aorta level. At these levels, MR had an excellent repeatability (ICC > 0.99), with a tendency to overestimate (4.6 ± 2.4% to 9.4 ± 7.0%). Quantification of AR by MR velocity mapping in the presence of a THV was accurate, precise, and repeatable in this pilot study, when corrected for the systematic error and when the best MR slice position was used. Confirmation of these results in future clinical studies would be a step forward in increasing the accuracy of the assessment of paravalvular AR severity.

OBJECTIVE: To investigate the feasibility of subtractionless first-pass single contrast medium dose (0.1 mmol/kg) peripheral magnetic resonance angiography (MRA) at 1.5 T using two-point Dixon fat suppression and compare it with conventional subtraction MRA in terms of image quality. METHODS: Twenty-eight patients (13 male, 15 female; mean age ± standard deviation, 66 ± 16 years) with known or suspected peripheral arterial disease underwent subtractionless and subtraction first-pass MRA at 1.5 T using two-point Dixon fat suppression. Results were compared with regard to vessel-to-background contrast. A phantom study was performed to assess the signal-to-noise ratio (SNR) of both MRA techniques. Two experienced observers scored subjective image quality. Agreement regarding subjective image quality was expressed in quadratic weighted κ values. RESULTS: Vessel-to-background contrast improved in all anatomical locations with the subtractionless method versus the subtraction method (all P < 0.001). Subjective image quality was uniformly higher with the subtractionless method (all P < 0.03, except for the aorto-iliac arteries for observer 1, P = 0.052). SNR was 15 % higher with the subtractionless method (31.9 vs 27.6). CONCLUSION: This study demonstrates the feasibility of subtractionless first-pass single contrast medium dose lower extremity MRA. Moreover, both objective and subjective image quality are better than with subtraction MRA. KEY POINTS: • MRA is increasingly used for vascular applications. • Dixon imaging offers an alternative to image subtraction for fat suppression. • Subtractionless first-pass peripheral MRA is possible using two-point Dixon fat suppression. • Subtractionless peripheral MRA is possible at 1.5 T a single contrast medium dose. • Subtractionless first-pass peripheral MRA provides good image quality with few non-diagnostic studies.

BACKGROUND: Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disease characterised by motor neuron degeneration. How this disease affects the central motor network is largely unknown. Here, we combined for the first time structural and functional imaging measures on the motor network in patients with ALS and healthy controls. METHODOLOGY/PRINCIPAL FINDINGS: Structural measures included whole brain cortical thickness and diffusion tensor imaging (DTI) of crucial motor tracts. These structural measures were combined with functional connectivity analysis of the motor network based on resting state fMRI. Focal cortical thinning was observed in the primary motor area in patients with ALS compared to controls and was found to correlate with disease progression. DTI revealed reduced FA values in the corpus callosum and in the rostral part of the corticospinal tract. Overall functional organisation of the motor network was unchanged in patients with ALS compared to healthy controls, however the level of functional connectedness was significantly correlated with disease progression rate. Patients with increased connectedness appear to have a more progressive disease course. CONCLUSIONS/SIGNIFICANCE: We demonstrate structural motor network deterioration in ALS with preserved functional connectivity measures. The positive correlation between functional connectedness of the motor network and disease progression rate could suggest spread of disease along functional connections of the motor network.

PURPOSE: To assess the validity and the direct, short-term, and long-term reproducibility of renal blood flow (RBF) measurements with phase-contrast (PC) magnetic resonance (MR) imaging. MATERIALS AND METHODS: In 20 healthy volunteers, RBF measurements were repeated with and without repositioning. Internal validity was assessed by comparing the total RBF with the difference in aortic flow above and below the renal arteries. In 19 healthy volunteers, RBF measurements were performed at two different occasions. In 40 healthy volunteers, RBF measurements were performed to assess normal values as a function of age. Analyses were performed according to Bland and Altman. RESULTS: The technical success rate ranged from 78% to 85%. Total RBF and the difference in aortic flow rates showed good agreement (Pearson correlation coefficient, 0.72; P = .002). Directly repeated measurements had a mean difference of 54 mL/min in total RBF with a coefficient of variation (CV) of 17%. For repeated measurements with repositioning, the mean difference in total RBF was 74 mL/min (CV, 23%). Repeated measurements on different occasions showed a CV of 20%. The mean total RBF of the 40 healthy volunteers was 838 mL/min +/- 244 (SD). CONCLUSIONS: RBF measurement with PC MR has a success rate greater than 75%. The demonstrated internal reliability of this method and fair reproducibility of the flow parameters is crucial for further studies of the renal artery with MR imaging.
